For requests and information, please use one of the forms below. All forms can be printed, or you may stop in the office during business hours to pick up. Forms MUST be returned to box on exterior of Lorraine City building at 238 N Main St.

Completed forms will be given to city council to handle at the next regularly scheduled city council meeting. Form must be filled out in its entirety, or it will not be addressed.
